Job Description
The Social Services Coordinator provides post-hospital care services to homeless individuals transitioning out of acute care, applying professional experience and judgment to tasks such as needs assessment, discharge planning, and community engagement. This role focuses on maintaining the health and stability of patients by providing resources and support. It requires flexibility and an ability to manage a caseload while adhering to organizational policies.
Requirements
- Timely complete Needs Assessment and Care Plans.
- Coordinate discharges and locate housing.
- Maintain up-to-date CRM database.
- Perform emergency referrals and coordinate with care partners.
- Maintain accurate case records.
- Practice harm reduction and manage guest searches.
- Adhere to all funder and company policies.
- Manage caseload of 25+ guests.
- Adapt to schedule changes.
- Triage and intervene during guest behavioral concerns.
