Job Description
The Neighborhood SHOPP Case Management Program aims to maintain and assist homebound older adults living independently in the community and reduce social isolation. The Case Worker will serve as a valuable resource to supplement staff resources and meet the organization's needs.
Requirements
- Baccalaureate Degree in Social Work from an accredited school
- Experience in a social service setting
- Strong understanding of the importance of volunteer programs and their impact on nonprofit organizations
- Ability to work with a diverse population and showcase the ability to work independently and collaboratively
- Bilingual skills in Spanish is recommended
- Computer literacy is mandatory, and knowledge of STARS is also helpful
