RN Clinical Manager - Hospice

New Age Hospice is seeking a Clinical Manager – Hospice to oversee clinical services and lead an interdisciplinary team.

Requirements

• Provide leadership and supervision to clinical staff
• Ensure timely and appropriate patient admissions, care planning, and service delivery
• Monitor and manage clinical performance metrics
• Conduct regular IDT and education meetings
• Oversee documentation practices
• Participate in the hiring, training, evaluation, and professional development of clinical staff
• Collaborate with leadership to support census growth and operational efficiency
• Serve as a clinical resource and mentor to staff
• Respond to patient and family concerns
• Participate in on-call rotation and provide clinical support as needed
