Job Description
This Role - Case Manager for Adult Mental Health Services provides case management and clinical support for individuals with mental illness, substance use disorders, and developmental disabilities and their families. The role involves counseling, crisis intervention, assessment, ISP development, psycho-education, and referrals to clients and families, and collaborating with other service providers. It also requires maintaining working relationships with service providers.
Requirements
- High School Diploma
- Registered as a QMHP (requiring registration and ongoing training)
- At least one year of experience working with individuals with mental illness, substance use disorders and/or developmental disabilities
- Experience in a treatment team
- Registration as a QMHP
- Maintaining QMHP status
- Maintaining required trainings/CEUs
