Care Coordinator

Newport Mental Health is seeking an experienced Care Coordinator to actively engage clients in their care coordination, helping them achieve positive outcomes. This role is integral to the client’s current care service, offering complementary and integrated support to the team. It's a grant-funded position focused on improving the lives of clients in Newport County.

Requirements

• Minimum of 1-2 years of experience in the Human Services field.
• High School Diploma or equivalent required, Associate Degree in a related field preferred.
• Exceptional verbal and writing skills.
• Self-motivated and independent.
• Valid Driver’s License with reliable transportation.
