Job Description
This position provides direct support to individuals with disabilities in the East area through Person-Centered Planning and Positive Behavior Supports. The role involves providing person-centered activities, supervising individuals, and assisting with daily living skills, including meal preparation, financial management, and community integration. Supervisory responsibilities are limited, focused on implementing Individual Support Plans and providing direct instruction.
Requirements
- Understand and implement Individual Support Plans (ISP's).
- Understand and implement Behavior Support Plans (as applicable).
- Administer medication as prescribed by the healthcare provider.
- Read, comprehend, and apply information from various documents and reports.
