Job Description
Case Management Director - RN or LCSW at Northeast Georgia Health System in Gainesville, GA. Coordinates case management and social work services, develops and implements case management education, and works closely with senior management and Providers to manage patient services and improve care continuity and patient outcomes.
Requirements
- RN or LCSW licensure
- Bachelor's Degree (Master's Degree required for Social Work candidates)
- Five or more years of experience in hospital clinical/financial operations and management
- Experience in at least one major component of Case Management, i.e. Discharge Planning, Clinical Social Work; previous Case Manager
- Experience working with third party payers and Physicians
