Physician Services Behavioral Health Care Coordinator

Optimal Care is a clinician-owned and operated company that provides Physician Services, Home Health, and Hospice Care. We are looking for a Behavioral Health Care Coordinator to process referrals, schedule patient visits, and manage the Behavioral Health Care Registry.

Requirements

• High School Graduate or GED equivalent
• Excellent verbal and written communication skills
• Basic computer skills
• Ability to read maps with general knowledge of the demographic area
