Job Description
Palm Primary Care is seeking a value-based Primary Care Nurse Practitioner to join their premier team in Ft. Worth, TX. Requirements
- Have at least 2-3 years' experience in the primary care/value-based care setting
- Must have an active and unrestrictive TX APRN license
- Must have a DEA license or eligible to obtain one within 2 months of hiring
- Strong understanding of chronic disease management, preventative care, and quality metrics in a value-based care model
- Excellent documentation and patient relationship-building skills
- Proficient with EMR systems (preferred)
