Job Description
This Health Care Guide I position provides support to HealthPlan members and their families, assisting them with benefits coordination and connection to internal and external resources. The role involves interacting with various care coordination team members, conducting assessments, and providing excellent customer service to both members and internal stakeholders. The Health Care Guide I role is crucial for coordinating care in a timely and effective manner.
Requirements
- High school diploma or equivalent
- Two (2) years of experience working in a health care setting (recommended)
- Experience in case management or care coordination preferred
- Knowledge of PHC and basic concepts of managed care
- Valid California driver’s license and current automobile insurance
