Job Description
PATH is seeking a Case Manager to assist clients in breaking the cycle of homelessness by utilizing a "whatever it takes" approach. This role supports clients through all phases of homelessness, including on-street, interim, and transitional housing, and utilizes services to connect them with healthcare, social services, and supportive resources. The position focuses on individual supportive services and fostering client agency in achieving housing goals.
