We are seeking a Case Manager to assist clients in breaking the cycle of homelessness. The Case Manager will conduct intake and individualized needs assessments, develop Individualized Service Plans, and provide referrals and linkages to resources. The ideal candidate will have analytical skills, attention to detail, and the ability to work effectively with others.
