Clinical Case Manager - Veterans Affairs Supportive Housing (JR 5048)

We are a mission and values driven organization that helps people find a place to live. As a Clinical Case Manager, you will provide case management and supportive services to program participants to assist them with breaking the cycle of homelessness and reintegrating into the community. You will work with clients and other staff to develop a Housing Stability Plan that addresses their short- and long-term goals.

Requirements

• Master's level credentials (MSW or MFT)
• Experience in homeless or permanent supportive services
• Demonstrate knowledge of and experience with advanced and evidence-based interventions
• Proficiency with computers and technologies used for work outputs
• A non-judgmental and respectful approach when working with individuals who present for services
• Professionalism and mature judgement
• High level of motivation and self-starting demeanor
• Flexibility, adaptability, and capability to work effectively in a fast-paced, professional environment
