Job Description
PATH is seeking Case Managers for its ICMS Program, focusing on identifying and supporting individuals experiencing homelessness and with chronic conditions. The role involves providing case management services, developing treatment plans, and collaborating with clients to achieve stable housing and maintain health. This position focuses on assisting individuals with acute cases and has a focus on supportive and healing services.
Requirements
- Maintain and execute confidential information according to HIPAA standards
- Demonstrate knowledge of or experience with evidence-based case management techniques
- Possess high level of tolerance and understanding for individuals with urgent and multiple case management and health needs
- Work independently as well as on a team
- Exercise mature judgment and motivation
- Excellent communication skills
