Pathways Home Health, Hospice and Private Duty seeks a Registered Nurse Case Manager - Home Health to provide skilled nursing care to patients and their families in the Mountain View - Redwood City area.
Requirements
- Current California RN license
- Current CPR Certificate
- Minimum of one year of nursing experience required
- Home Health Experience
- Current California driver's license and automobile insurance
