The Care Team Nurse is an integral part of the health care team, promoting and facilitating excellent health care for patients in accordance with established nursing standards. This position works as a member of a care team to coordinate patient care and support patients and families in self-management, self-efficacy, and behavior change.
Requirements
- RN, graduate of an accredited school of nursing
- Two to three years of related experience or training
- Clinic experience strongly preferred
- EMR experience helpful
- Current professional license to practice in the State of Texas
- CPR Certified
- Bilingual in English/Spanish required
- Strong written and verbal communication skills, including reflexive listening
- Skill in public relations and group presentations
- Basic counseling skills and health education knowledge, including an understanding of human anatomy and physiology
