Job Description
As an RN Home Health Case Manager, you will play a critical role in delivering high-quality care to our patients in their homes. You will assess and evaluate patient needs to develop and implement individualized care plans, coordinate and oversee the delivery of patient care, and serve as a primary contact for patients and their families regarding care and treatment.
Requirements
- Current RN license in the state of practice
- Minimum of 2 years nursing experience, preferably in home health or community health
- Strong assessment, critical thinking, and clinical skills
- Excellent communication skills both verbal and written
- Ability to work independently and as part of a multidisciplinary team
- Familiarity with electronic medical records (EMR) systems
- Compassionate and patient-centered approach to care
