Job Description
As a Home Health Case Manager, you will deliver hands-on patient care in patients' homes, coordinate services of other healthcare providers, communicate with health insurance companies, and oversee visiting nurses. You will develop personalized care plans and collaborate with nurses and therapy professionals.
Requirements
- Kansas or Compact Registered Nursing and Driver's License.
- Strong organizational and self-management skills
- Strong and compassionate customer service skills
- Valid driver's license
