Job Description
Pioneer Human Services is seeking a (on-call) Health Coordinator to support the mission of providing access to quality and cost-effective integrated health services to community members. The role involves coordinating client health needs, assessment, and referral to appropriate providers, utilizing motivational interviewing techniques and maintaining accurate medical documentation. The position offers a supportive and values-based work environment within a national organization.
Requirements
- Assess individual risk, needs, and barriers using validated tools and techniques
- Motivational Interviewing (MI) and skill development
- Interprets and explains program policy and goals to each client
- Assists licensed nurses with intakes
- Coordinate client intake and discharge
- Maintain accurate documentation of service
- Ensure all necessary notifications are completed
