Job Description
Porter Cares is expanding into South Chicago and seeking Nurse Practitioners to conduct in-home health risk assessments. The role involves evaluating medical history, performing physical exams, and identifying diagnoses for care management, all within a consumer-centered healthcare ecosystem focused on transparency and personalized care.
Requirements
- Valid unencumbered Illinois NP license
- 3 years of patient care experience required
- Strong sense of empathy, compassion, and friendliness
- Exceptional communication skills
