Job Description
Porter is expanding in Illinois and seeking Nurse Practitioners to conduct in-home health risk assessments. The role involves evaluating patient history, performing physical exams, and identifying diagnoses to support care management. Requirements
- Valid unencumbered Illinois NP license
- 3 years of patient care experience required
- Experience in primary care/adult/geriatric preferred
- EMR skills preferred
- Strong sense of empathy, compassion, and friendliness
- Exceptional communication skills, both written and oral
