The Physical Therapist provides high-quality physical therapy services to older adults across home and center settings within a multidisciplinary care model. This position combines clinical expertise with compassionate care to promote independence and well-being in every participant.
Requirements
- Perform physical therapy evaluations, treatments, and follow-up assessments for PACE participants
- Develop individualized therapy plans addressing mobility, balance, strength, and functional goals
- Collaborate with the Interdisciplinary Team (IDT) to support coordinated care planning and rehabilitation goals
- Educate participants and caregivers on home exercise programs, durable medical equipment (DME), and safety practices
- Conduct home visits as needed and document therapy services, participant progress, and outcomes
- Contribute to quality assurance, infection control, and safety programs
- Assist in ordering and maintaining DME inventory
- Provide subject matter expertise for staff education, program development, and therapy standardization
- Participate in staff meetings, clinical audits, and data collection initiatives
