The Mental Health Targeted Case Manager is responsible for the case management of up to 25 clients, creating and implementing individualized care plans, and linking families to needed resources. The position requires strong leadership and organizational skills, excellent communication and conflict resolution skills, and the ability to understand and relate to clients from diverse backgrounds.
Requirements
- Bachelor's Degree in a human services or related field
- One year of experience in community-based services or a Master's Degree in a Human Services Field
- Must be at least 21 years of age
- Must have a valid Driver's License and be insurable
- Must have excellent communication and conflict resolution skills
- Technical requirements include proficiency with Microsoft Word, Excel, PowerPoint, and other applications used by the organization or regulatory agencies
