Sacramento Behavioral Healthcare Hospital (SBHH) is a new facility located near downtown Sacramento. As the first newly constructed psychiatric hospital in Northern California in more than 30 years, this hospital offers 117 inpatient acute psychiatric beds, an outpatient treatment center, and a procedures clinic providing electro-convulsive therapy (ECT) and other interventional psychiatry services. We offer a full range of individualized treatment services, adults, and senior adults, including treatment for mental health and co-occurring (mental health and substance abuse) disorders. Additional services include mental health and substance abuse screening, assessment, crisis intervention, consultation, education, and individual and group counseling.
Job Description
This role is a Case Manager providing quality case management and discharge planning services to patients and their families. The CM will ensure linkage to mental health/recovery services, facilitate communication with County Liaison, and provide daily placement documentation. Responsibilities include administrative tasks, ensuring follow-up appointments, and verifying patient identity.
Requirements
- Knowledge and Experience: Minimum of a Bachelor’s Degree in Psychology, Social Work, or closely related field
- Current CPR certification or during initial orientation period
- Current HWC (Handle with Care) certification or during initial orientation period
- Familiarity with continuum of care and clinical terminology
- Strong organizational skills
- Good judgment and problem solving skills
- Demonstrated commitment to working collaboratively
