Job Description
The Case Planner works with families to support safety, permanency, and well-being for children and youth. The Case Planner develops and implements service plans, addresses barriers, and assesses progress to achieve goals.
Requirements
- Bachelor's degree in Social Work or related field
- Approved background clearances and fingerprinting
- Excellent oral and written communication skills
- Proficiency in computer skills, including Microsoft Office, Excel, and Connections
- Bilingual abilities are strongly encouraged
- Valid NYS Driver License
