Case Manager

Seasons Center is seeking a full-time Case Manager to help individuals with mental and behavioral health challenges access services and support. The role involves coordinating resources, advocating for clients, and ensuring treatment plans are followed. This position could be based in Sheldon, IA, or Sioux Center, IA.

Requirements

• Bachelor’s degree in social work, human services, or a related field
• Valid driver’s license and reliable transportation
• Active Iowa Bachelor’s of Social Work (BSW) license (preferred)
