PACE Center Intake Coordinator

Seen Health is revolutionizing senior care through the PACE model, and we're looking for a PACE Center Intake Coordinator to facilitate enrollment, ensure compliance, and coordinate care between healthcare providers.

Requirements

• Bachelor's degree in healthcare, social work, nursing or related field
• Minimum of two years of experience in healthcare service coordination or case management
• Knowledge of Medicare and Medicaid regulations preferred
• Excellent communication and interpersonal skills
• Ability to work independently and as part of an interdisciplinary team
• Strong organizational and time-management skills
• Ability to maintain confidentiality and exercise good judgment
• Proficiency in Microsoft Office and electronic health records (EHR) systems
