We are seeking a Case Manager to join our team at Select Specialty Hospital in Jackson, TN. The Case Manager will be responsible for utilization reviews and resource management, discharge planning, treatment plan management and financial management, while also completing medical record documentation. The ideal candidate will have a current licensure in a clinical discipline, previous experience in an inpatient hospital setting, and experience in care management and discharge planning.
