Job Description
ServiceNet is seeking a compassionate Case Manager to join their team at Our Friends Shelter in Pittsfield, MA. This role focuses on assisting individuals and families experiencing homelessness in building the foundation for long-term stability through direct support, advocacy, and collaboration with various resources. The position requires providing essential support and ongoing guidance as guests work toward permanent housing.
Requirements
- Conduct comprehensive intakes and assessments for new shelter guests
- Support and advocate for individuals and families in shelter or transitional housing
- Help participants access and follow through with treatment, medical, vocational, housing, and community-based resources
- Hold weekly meetings with guests to review service/treatment plans, set goals, make referrals, and monitor progress
- Foster a positive and supportive residential environment
- Maintain clear, accurate case management and progress notes
- Valid driver's license and acceptable motor vehicle record
