Job Description
Soleo Health is seeking a Clinical Transition Coordinator to support patients moving between hospital and alternative care settings. This role involves coordinating patient care, providing patient and family education, assisting in transfers to alternative care sites, and collaborating with physicians and other healthcare providers. The Coordinator will also manage referral sources and ensure smooth transition for patients, working with case managers and sales teams.
Requirements
- Registered Nurse required
- Minimum five (5) years of clinical experience, home health field or management experience preferred
- Knowledge of hospitals, skilled nursing/rehab facilities, case management and discharge planning service needs
- Advanced practice expertise to identify and implement improvement processes
