The VNA of Manchester and Southern New Hampshire provides 24/7 in-home support and services to patients in Manchester and the surrounding communities. We are committed to helping our patients navigate the challenges they face at home while recovering from surgery or coping with life-limiting illnesses or physical disabilities.

Requirements

• Delivers home health care to client/patients of all ages and families
• Assesses patients' physical and psychosocial status and the home environment
• Reviews all medical data prior to delivering patient care
• Documents and communicates patient status and assesses needs to the Home Care Nurses
• Obtains prior authorizations and documents adequately to ensure prevention of write-offs
• Explains insurance coverage and cost of care appropriately to patients/significant others
