Case Manager

Southwest Behavioral and Health Services is seeking a dedicated and empathetic Case Manager who can help us fulfill our mission. We are seeking someone who enjoys coordinating with people, ability solve problems, organized and excellent communication skills.

Requirements

• Intake screenings
• Completing annual behavioral health assessments
• Facilitating CFTs and ARTs
• Facilitating groups
• Identifying gaps in care for our members
• Completing SMI and SED evaluation
