Job Description
Strive Health is transforming the kidney care system by integrating early identification, engagement, and comprehensive coordinated care. They seek a Nurse Practitioner to manage patients with Chronic Kidney Disease (CKD) and Esiotensin-Keeper Disease (ESKD) through advanced nursing care, advocating for patients, and delivering high-quality patient experiences. The role requires collaboration with interdisciplinary teams and a commitment to best practices in chronic kidney care.
Requirements
- Master’s degree in Nursing or related field
- 2+ years of experience as a Family Nurse Practitioner (NP), Primary Care Nurse Practitioner (NP), or internal medicine, cardiology, nephrology, or case management experience
- Current state-specific licensure required
- National Board Certification
- DEA license required or eligible to obtain
- BLS/CPR Certification
- Efficient transportation with an active driver’s license
- Minimum Internet Speeds: 3.8Mbps/3.0Mbps (Latency <60 ms)
