Requirements
- Provide initial and ongoing assessment and interpretation of patients
- Develop, coordinate and update an individualized plan of care
- Establish and maintain a therapeutic relationship with patient or families
- Utilize data obtained from the patient and their family and other health care providers to evaluate the plan of care
- Supervises and evaluates care provided by direct care assistive and support staff
- Maintain patient confidence and protects operations by keeping information confidential
