Job Description
The Homeless Prevention Coordinator (HP Coordinator) for the S:US Supportive Services for Veteran Families (SSVF) program plays a key role in helping veteran families meet their housing goals. This position involves providing comprehensive support, including assistance with housing, benefits, legal aid, and access to resources. The HP Coordinator conducts screenings, facilitates access to resources, and supports veteran households with outreach and case management.
Requirements
- Assist veteran households in achieving their financial goals, planning for a new housing situation.
- Conduct veteran home studies for potential program enrollment.
- Manage program tracking and coordination of case updates.
- Navigate veterans to a variety of resources, including Homebase and legal aid.
- Maintain case files and facilitate referrals to internal and external services.
