The Registered Nurse will provide mental health support to individuals transitioning from hospitals back into the community. They will use the Critical Time Intervention model to connect clients with community resources and provide case management, referrals, and counseling. This role involves documenting medical records, administering medications, and following prescribed regimens. The position requires limited on-call coverage.
