Community Health Worker (Bilingual)

Swope Health is seeking a compassionate and mission-driven Community Health worker to assist individuals and families in enrolling in public benefits and connecting them to critical resources.

Requirements

• High school diploma or GED equivalent
• Bilingual (English/Spanish)
• Certification or formal training as a Community Health worker highly preferred
• Certified applications counselor (CAC) certification highly preferred
• Patience and ability to establish rapport with this population
• Flexible schedule and transportation
• Knowledgeable about diverse clientele
• Ability to write clearly and distinctly
