Job Description
The Behavioral Health Specialist Case Manager will provide strength-based case management services to individuals with a history of being chronically unhoused and behavioral health conditions. The position will contribute to the PSH Care Team by promoting wellness and housing stability and retention.
Requirements
- High school diploma with three years of case management or care coordination experience (bachelor's degree is preferred)
- Valid driver license in the state of Ohio with access to an insured vehicle for client transportation
- Ability to form effective therapeutic relationships to engage clients in the mutual assessment of problems and goal setting
