Outreach Specialist

This Outreach Specialist role involves reaching out to participants to schedule appointments, re-engaging those who have missed appointments, and engaging/reengaging those who have not. Responsibilities include conducting telephone/email outreach, identifying barriers to participation, and communicating with clinical staff and other departments.

Requirements

• Reach out to participants and re-engage those who fail to report
• Conduct escalating outreach to participants
• Identify and assist with mitigating barriers to program participation
• Communicate case notes to the clinical staff
• Post case notes into HRA and Fedcap management systems
