Job Description

Requirements
- Provide rounding and managing clinical care at assigned SNFs.
- Prescribe medications and treatments for SNF residents.
- Provide patient-centered, comprehensive primary care services in SNFs.
- Complete accurate and timely documentation in the EHR system.
- Reliable transportation required.
