Job Description
The Department of Family Medicine and Community Health is seeking a full-time Case Worker to coordinate services for patients, including providing information on professional development opportunities, quality improvement strategies, and counseling to improve client's social and psychological conditions. The Case Worker will also provide assistance to those experiencing difficult situations by helping them find community resources/services that they need.
Requirements
- Bachelor’s degree in relevant field
- Minimum 1 year of relevant experience
