Part Time Licensed Clinical Social Worker (LCSW) - PC/MH Integration

Valor Healthcare is seeking a passionate Licensed Clinical Social Worker (LCSW) for a part-time position at their Community Based Outpatient Clinic in Columbia, TN. The role focuses on integrating primary care and mental health to expand access to services, assessment, and treatment, particularly for veterans. This position is 16 hours/week, offered on a weekday schedule with flexibility on the schedule.

Requirements

• Participate in staff meetings regarding clinical. administrative, and staff development issues.
• Perform initial and follow-up evaluations, including assessments.
• Provide treatment services, including individual therapy.
• Perform crisis intervention and case management.
• Maintain confidentiality of veteran data.
• Perform case management and coordination of care.
• Educate Veteran and family members on psychosocial needs.
• Pass required VA training.
• Meet compliance requirements.
