Clinical Case Manager, Urgent Care

This Clinical Case Manager role supports student mental health needs in a supportive and collaborative environment. The role involves providing case management, scheduling referrals, coordinating with other resources, assisting with discharge planning, and providing student support in a rapid evaluation setting. The role is part of the Student Care Network team, working with campus and community partners to ensure student success.

Requirements

• Track no-shows/cancellations of high-risk clients.
• Schedule off-campus care referrals requested by UCC clinicians.
• Develop effective working relationships with the Vanderbilt community.
• Assist with coordination of care.
• Monitor and update UCC Clinicians of midterm deficiencies.
