Job Description
Vanna Health is seeking an experienced Registered Nurse (RN) to join our community care team. The ideal candidate will have a passion for serving individuals with mental illness and chronic physical health conditions, and will be responsible for promoting continuity of care, education, and population level program development.
Requirements
- Minimum of 3 years working with adults with mental illness and substance use disorders in community based settings
- Minimum of 1 year of experience working as a medical nurse or working as a nurse in inpatient or outpatient medical settings
- Experience working in and around the community of Maricopa County
- Ability to work in a team
- Willingness to learn how to work with Google, Slack, and Vanna EHR tools
- Basic computer and phone skills
- A valid driver’s license
