Nurse Practitioner

Records physical findings, formulates plan and prognosis, prescribes or recommends drugs or treatment, monitors patient progress and reports to supervising physician. Collaborates with hospice employees and volunteers. Requires completion of accredited Nurse Practitioner program and state licensure.

Requirements

• Minimum 2 years experience in hospice, home health or community health in the past 5 years
• Ability to assist in lifting and transferring patients as required
• Close vision, distance vision, and ability to adjust focus
