Case Worker II (School Based Health Center) - Pediatrics Adolescent Medicine

The Case Worker II will provide youth-centered case management/referral services for high school age youth as part of a school-based health center. The position will coordinate with other behavioral health/health staff members of the SBHC multidisciplinary team and closely with school faculty and leadership to implement services within the school setting.

Requirements

• Master’s degree or combination of education and/or experience may substitute for minimum education
• Basic Life Support certification must be obtained within one month of hire date
• Case Management, Clinical Evaluations, Community Resources, Computer Literacy, Effective Written Communication, Family Support, HIPAA Compliance, Oral Communications, Patient Care, Patient Education, Prevention Programs, Referral Coordination, Team Collaboration, Team Communication
