Home Care Coordinator Supervisor

The Home Care Coordinator Supervisor collaborates closely with a team of Home Care Coordinators, overseeing their teamwork with other members of the Home Services team, as well as with other organizations and diverse community members.

Requirements

• Healthcare or medical licensure or equivalency, with an additional three (3) years of professional experience
• Bachelor’s degree preferred
• Minimum of three (3) years of case management or nursing experience in a home setting with frail or elderly population
