Social Worker

We are seeking Social Workers who have completed their MSW program and are interested in working with our vulnerable, senior population. As a member of our interdisciplinary team, you will work with clinical and non-clinical members to provide quality comprehensive primary care and social services to keep seniors living safely in the community.

Requirements

• Master’s degree in social work (MSW) required
• Minimum of one (1) year of experience working with a frail or elderly population preferred
• Experience coordinating and facilitating care conference meetings preferred
• Experience assisting people with behavioral health or substance abuse issues preferred
