Patient Care Coordinator

The Patient Care Coordinator is responsible for providing compassionate care to the frail population who suffer from chronic and acute illnesses or injuries. The successful candidate will employ critical thinking and decision making, be comfortable with technology, have flexibility, and love working with patients and a collaborative interdisciplinary team.

Requirements

• High school graduate or GED
• 2+ years of experience working for a healthcare practice or hospital
• EMR documentation experience preferred
• MA Certification-Preferred
