Job Description
PURPOSE STATEMENT: Provide psychosocial support to individuals, families, and groups to cope with chronic or acute illnesses. Services include advising caregivers, providing information/counseling, and facilitating case management. This role involves evaluating patient needs and coordinating referrals to community resources.
Requirements
- Collaborate with other professionals to evaluate patients.
- Advocate for clients/patients to resolve crises.
- Refer patient to community resources for recovery, support, and services.
- Investigate child abuse cases.
- Counsel clients/patients in individual and group sessions.
- Monitor client progress according to treatment plans.
